1.1 These terms apply to all Urban Health Care gift vouchers and gift cards (“vouchers”), whether bought online, by phone or at our clinic. By buying or using a voucher, you agree to these terms.
1.2 Vouchers are issued by Lorimell Limited, trading as Urban Health Care, a company registered in England and Wales (company number 09555380). Our registered office and clinic are at 301–305 High Street, Croydon, England CR0 1QL. Regulated clinical services at the clinic are provided by SJLD Ltd, which is registered with the Care Quality Commission (CQC).
1.3 In these terms, “we”, “us” and “our” mean Urban Health Care. “Purchaser” means the person who buys the voucher. “Holder” or “you” means the person who presents the voucher for use.
1.4 Your treatment itself is also covered by our general Terms & Conditions, which apply alongside these terms.
2.1 Vouchers are available for a monetary value. [Where we offer them, vouchers may also be sold for a named non-prescription service, such as a massage, facial or dental hygiene appointment.]
2.2 Vouchers are issued digitally and sent by email to the address given at checkout. Delivery is usually instant. Please check your junk or spam folder first; if your voucher still hasn’t arrived, email info@urbanhealthcare.co.uk. If you would prefer a printed voucher, you can collect one from reception at our clinic.
2.3 Please check the recipient’s details carefully. We are not responsible for vouchers sent to an incorrect email or postal address given by the purchaser.
2.4 Payment must be made in full at the time of purchase. Vouchers are not valid until payment has cleared.
2.5 Each voucher has a unique code. Treat it like cash: anyone who presents a valid code may use it.
3.1 Vouchers are valid for 12 months from the date of purchase. The expiry date is shown on the voucher.
3.2 Your treatment must take place on or before the expiry date. Booking an appointment before expiry for a date after expiry does not extend the voucher.
3.3 Any value not used by the expiry date will be lost. We cannot extend, refund or reissue expired vouchers, although we may do so at our discretion in exceptional circumstances (for example, if the clinic was unable to offer you an appointment before expiry).
4.1 Vouchers can be used towards treatments and services at Urban Health Care across our dental, medical, aesthetics and beauty & wellness services. They can only be used at our Croydon clinic.
4.2 Vouchers cannot be used for:
4.3 Please mention your voucher when booking and quote its code. Bring the voucher (printed or on your phone) to your appointment.
4.4 Vouchers can be used over more than one visit until the balance is used up or the voucher expires. The remaining balance is recorded against the voucher code.
4.5 If your treatment costs more than the voucher value, you pay the difference. If it costs less, the balance stays on the voucher; we do not give change in cash.
4.6 Vouchers cannot be exchanged for cash or credit, and are not refundable except as set out in section 7.
4.7 Unless stated otherwise, vouchers cannot be used alongside any other offer, promotion or discount, including our Friday special offers.
4.8 Vouchers may be passed on to someone else but must not be resold.
5.1 A voucher is a way of paying for treatment. It does not guarantee that a particular treatment will be provided.
5.2 Many of our services need a consultation first. Our clinicians will only carry out a treatment if they judge it clinically appropriate and safe for you. If a clinician decides a treatment is not suitable, you can use the voucher towards an alternative treatment or service.
5.3 Treatments involving prescription-only medicines (such as anti-wrinkle injections) can only be provided after a face-to-face consultation with a qualified prescriber. Vouchers are not sold for named prescription-only treatments, but can be used towards a consultation and any treatment that is prescribed afterwards.
5.4 Aesthetic and cosmetic treatments are only available to people aged 18 or over. Proof of age may be requested. Some other services also have minimum ages.
5.5 Where a treatment requires consent, the holder must give that consent themselves. A purchaser cannot consent on the holder’s behalf.
5.6 The holder must provide an accurate medical history. Treatment may be declined or postponed if relevant information is not disclosed.
5.7 Prices may change between the date of purchase and the date of treatment. The voucher is worth its stated monetary value, not a fixed treatment price, unless it names a specific service.
6.1 Appointments are subject to availability. We recommend booking well before your voucher expires, especially around Christmas and other busy periods.
6.2 If you need to cancel or rearrange, please give us at least 48 hours’ notice.
6.3 If you cancel or rearrange with less than 48 hours’ notice, or do not attend, the full cost of the booked treatment will be deducted from the voucher balance, in line with our cancellation policy.
6.4 If we need to cancel your appointment, we will rebook you at no cost and the voucher value will not be affected.
7.1 If you bought a voucher online or by phone, you have the right to cancel within 14 days of purchase and receive a full refund, as long as the voucher has not been used. This is your right under the Consumer Contracts (Information, Cancellation and Additional Charges) Regulations 2013.
7.2 To cancel, email info@urbanhealthcare.co.uk with your order number and voucher code. We will cancel the voucher and refund you using the original payment method within 14 days.
7.3 Only the purchaser can request a refund. Refunds cannot be paid to the holder if they are a different person.
7.4 After 14 days, or once any part of the voucher has been used, vouchers are non-refundable.
7.5 Nothing in these terms affects your statutory rights.
8.1 Please keep your voucher safe. If a digital voucher email is lost, we can resend it to the original email address on request from the purchaser.
8.2 We cannot replace vouchers that have been lost or stolen and already used by someone else. If you tell us promptly, we will try to block the code, but we accept no responsibility for any value used before we are told.
8.3 We may refuse a voucher that is altered or unreadable.
9.1 Vouchers given free, as a prize, as part of a promotion or at a discount may carry extra conditions, which will be shown on the voucher or in the promotion. Where they conflict with these terms, the promotional conditions apply.
9.2 Promotional and complimentary vouchers have no cash value, cannot be refunded and may have a shorter validity period.
10.1 We may refuse or cancel any voucher that we reasonably believe was obtained fraudulently, bought with a stolen payment card, copied, altered or resold.
10.2 If a payment is reversed or charged back, the voucher will be cancelled and any treatment paid for with it becomes payable.
11.1 If a service named on a voucher is no longer offered, you can use the voucher’s purchase value towards another treatment or service.
11.2 We may update these terms from time to time. The terms in force on the date the voucher was bought will apply to it, unless a change is required by law or regulation.
11.3 If Urban Health Care permanently closes, we will give holders reasonable notice to use their vouchers.
12.1 We use the purchaser’s and recipient’s details only to process the order, deliver the voucher and manage its use. We handle all personal information in line with UK data protection law and our Privacy Policy.
12.2 Health information given by the holder at their appointment is kept in their confidential clinical record and is never shared with the purchaser.
13.1 Our liability for any voucher is limited to its unused value. This does not limit our liability for death or personal injury caused by our negligence, for fraud, or for anything else that cannot be limited by law.
13.2 These terms are governed by the law of England and Wales. Any dispute will be dealt with by the courts of England and Wales.
